Thanks for visiting the Active Lives Survey page – if you complete the survey we’ll send you a £5 gift voucher which can be used at a range of
stores across the UK.
If you fill in the survey online you will be asked to provide an email address if you wish to receive a gift voucher. We will then
pass your email address to a third party supplier who will send you an email within seven days containing a personalised link to a website.
Once you have selected your gift voucher it will be emailed to you within ten working days. If you do not receive your voucher within ten working days please check your junk or spam folder in case the email containing the voucher was filtered out of your inbox.
Where can my digital voucher be used? If you fill in the online survey, you will be sent a link which can be used to select a £5 gift voucher of your choice from one of the following: Amazon, Argos, ASOS, B&Q, Costa, Currys PC World, Decathlon, H&M, Halfords, Harvester, Inspire Travel Card, iTunes, John Lewis, M&S, Nandos, National Book Tokens, National Trust, New Look, Nike, Odeon, Pizza Express, Tesco, The Entertainer, TK Maxx, Theatre Tokens or Toby Carvery.
I have difficulties redeeming my e-voucher, who do I contact? If you encounter any problems while trying to redeem your voucher, please in the first instance try using a different browser/device.
If the problem persists, you can email us at firstname.lastname@example.org or leave a voicemail message on 0808 129 6801 with your unique 14 digit reference number.
